As former chief medical officer of Alchem Laboratories Corporation, Malone has previously been involved in work on famotidine: in April 2020, Alchem and its subcontractor Northwell Health were awarded a $20.7-million government contract to test the drug in combination with hydroxychloroquine in patients with moderate to severe COVID-19, the Associated Press reported last summer.
Malone left that study and resigned from Alchem shortly after the contract was awarded, citing a difficult working environment, he told the AP and confirms in an email to The Scientist.

The severity, morbidity and mortality of COVID-19 must be re-emphasized to all, both young and old, as it spares neither. To avoid a catastrophic repeat of the initial surge, we recommend a population-wide intervention — a significant increase in the use of N95 masks — that might allow for a safer reopening of the U.S economy.
SARS-CoV-2 was thought to be primarily transmissible via large fluid-filled droplets generated by coughing or sneezing. Those droplets usually travel short distances before falling and will not reach another person practicing social distancing nor penetrate standard (cloth or surgical) masks.
However, it has recently been determined that a major mode of transmission of SARS-CoV-2 is via aerosol droplets, exhaled by presymptomatic, asymptomatic or symptomatic persons. These small aerosol particles remain airborne indoors for extended periods and can infect those nearby who inhale them into their lungs.
We believe that the lack of emphasis placed on the prevention of aerosol droplet transmission is a major contributor to the rising case numbers in many areas of our country.
What can be done to prevent the inhalation of these aerosols and therefore reduce the spread of SARS-CoV-2? The only mask that can prevent aerosol-size droplet inhalation is an N95.
Too few Americans wearing masks (write Kory and Mayo)
Here is the key: Standard masks, although only partially effective in blocking inhalation of aerosols compared with the near perfect blocking performance of medical grade N95 masks used in hospitals, are highly effective at trapping the large droplets exhaled by infected people.
These large particles downsize to aerosol size after emission when they undergo evaporative loss. Although standard masks are imperfect in both blocking or trapping, their combined performance when worn by both infected and noninfected persons leads to a low likelihood of transmission.
The key point is that, for standard masks to be effective, there needs to be near universal wearing of these masks by all persons when in any poorly ventilated, air-recirculated, confined indoor, or highly congested outdoor environment. Conversely, emphasizing mask wearing in fresh air outdoor settings has no epidemiologic support and thus makes little sense.
Unfortunately, in some parts of the United States, the proportion of citizens routinely wearing standard masks in at-risk environments is nowhere near what's required to prevent spread. Ranitidine was discovered in England in 1976, and came into commercial use in 1981. It is on the World Health Organization's List of Essential Medicines. It is still available as a generic medication via the internet. July 1 (Reuters) - A Delaware judge rebuffed a request by GSK (GSK.L), opens new tab and other drugmakers to appeal a ruling allowing more than 70,000 lawsuits claiming that the heartburn drug Zantac caused cancer to go forward.
The ruling by Judge Vivian Medinilla of the Delaware Superior Court means that the drugmakers, which also include Pfizer (PFE.N), opens new tab, Sanofi (SASY.PA), opens new tab and Boehringer Ingelheim, will have to ask the Delaware Supreme Court directly for permission to appeal. GSK said it already submitted its appeal to that court.
If the state high court declines to take the appeal, it will clear the way for the Zantac lawsuits to go to trial.
"Judge Medinilla resoundingly rejected GSK, Boehringer Ingelheim, Pfizer, and Sanofi's attempt to end run around the jury system in Delaware," said Jennifer Moore, a lawyer for the plaintiffs.
GSK in a statement said "the scientific consensus remains that there is no consistent or reliable evidence that ranitidine increases the risk of any cancer." Ranitidine is the active ingredient in the now discontinued drug.
Lawsuits began piling up after the U.S. Food and Drug Administration in 2020 asked manufacturers to pull the drug off the market over concerns that ranitidine could degrade into a cancer-causing chemical called NDMA over time or when exposed to heat.
The drugmakers say Medinilla should have kept the plaintiffs from introducing expert testimony that Zantac can cause cancer, as a federal judge did in 2022 in about 50,000 claims centralized in Florida.

Pepcid (famotidine) and Zantac (ranitidine) are both in the same family of drugs and are extremely similar. There are no large scale studies comparing their effectiveness against one another unfortunately. These drugs appear to be essentially identical in terms of effectiveness, adverse reactions and tolerability. In the few small studies that did compare them, they both showed similar healing rates for ulcers and a similar reduction in the symptoms of acid reflux.
Overall, most studies that were conducted conclude that there is no statistically significant difference in terms of effectives between Pepcid (famotidine) and Zantac (ranitidine).
Pepcid (famotidine) is a H2 blocker (histamine 2 receptor blocker) that is used to decrease acid secretion in the stomach in order to treat acid reflux, also known as heart burn, and to treat gastric ulcers. Famotidine is typically taken twice a day and does not have many side effects associated with it. Pepcid comes in dosages of 20 mg and 40 mg.
Zantac (ranitidine) is also a H2 blocker that is used to decrease acid secretion in the stomach. It is also used to treat ulcers and heart burn and is dosed twice daily. Zantac comes in dosages of 75 mg and 150 mg.
It is important to note that even though there is a significant difference in mg dose between the two drugs, that does not correlate to difference in potency. Different drug compounds require different dosages to have the required effect.
In terms of side effects, famotidine and ranitidine again are very similar. The side effects associated with them including dizziness, agitation, and constipation; however, these side effects are relatively uncommon and both drugs are very safe.
Both famotidine and ranitidine have the same mechanism of action and are dosed twice daily when used to treat heartburn over-the-counter. Famotidine works within 1-3 hours while ranitidine works within 2-3 hours. Both drugs will require a dosage adjustment if you have any issues with your kidneys (for example, if you have renal disease or receive dialysis).
Response to both drugs will vary by individual and it may take some trial and error to determine which works best for your symptoms.

Below is a list of projects from 2011-2016 that Robert Malone and I (RW Malone MD, LLC) have provided direct leadership and proposal management in. That is almost ten billion dollars in the last five years. We helped win 562 million in contract awards and nine billion in IDIQ awards. That is ten billion dollars that these companies may not have won with out involvement. That is a whole lot of really important research that wouldn't have happened if it weren't for us, particularly Robert. It is kind of mind blowing.
We are still looking for funding for our Zika clinical trials but the US Gov appears to have decided to not fund any anti-viral clinical research (and we are just about ready to go). So, I am on the hunt for our next big project. I was told by a "big" project management firm that other day, we we work for about a tenth of what they do... But our success rate is over 95%, while theirs is below 50%.
We work our asses off, we work smart, we don't really advertise, we have clients that have been with us for years and we haven't raised our rates in a decade. So, as I am completely discouraged with our government again over their response to Zika, I have to remember our wins. Because we are doing something right. We may not fund big conferences, or big dinners at awards nights but we bring in the contracts for our clients, we write solid scientific proposals that get funded and we don't brag (much)...

Somewhere in the presentation he made two statements that I want to insert at this time. I don't remember just where they were made, but they're valid in terms of the general overall view.
One statement: "People can carry in their minds and act upon two contradictory ideas at one time, provided that these two contradictory ideas are kept far enough apart."
And the other statement is, "You can know pretty well how rational people are going to respond to certain circumstances or to certain information that they encounter. Somewhere in this connection, then, was the statement admitting that some scientific research data could be - and indeed has been - falsified in order to bring about desired results. And here was said, "People don't ask the right questions. Some people are too trusting."

It could be brought about in other ways. Acceptance of the U.N . at that time was seen as not being as wide as was hoped. Efforts would continue to give the United Nations increasing importance. People would be more and more used to the idea of relinquishing some national sovereignty.
